MAP Test Refusal ("Opt Out") Letter 2024 - ONLY for Grades 6 - 10
This form was created by parent members of ICE PAC (ICE Parent Action/Education Advocacy Committee), not by the ICE administration, faculty, or staff. All form entries will be delivered to Institute for Collaborative Education Principal Peter Karp.
In submitting this form, I refuse permission for my child to participate in the NWEA MAP test. I understand that the Institute for Collaborative Education has already, or will soon, assess my child using a teacher-created diagnostic assessment, in accordance with the New York Performance Standards Consortium. The decision to refuse in no way reflects on the hard work and talent of the teachers, administration, or staff at our school.
Please complete one separate form for EACH child in your family enrolled in grades 6 - 10 at our school.
